Based on the NeelTec pictures above and info from Fiero and MR2 forums the following should be all the materials needed to complete the quad headlight conversion.
Light assembly:
(2) HL68137 -- H9 halogen projector low beam (DOT compliant)
(2) HL68136 -- H9 halogen high beam (DOT compliant)
Sheet metal for bracket fabrication
Plastic or fiberglass for fabrication of lens surround
This drawing is of Hella part #97300 mounting plate for 90mm modular lights. This provides dimension for making an adapter plate like NeelTec did.

16 AWG (or higher) Wire
(4) Hella HL87193 H9 female connectors (or equivalent)
(2) Hella CPH4CM H4 male connectors (or equivalent)
This is a rough wiring diagram of the adapters.
All that's left is modifying the Headlight lifting cam and arm to raise the headlight to the proper height. This will require fabricating a new, shorter cam to bolt to the motor and shortening the arm so that the motor rotation causes less vertical movement than the original factory setup. The sleepy eye mod may be a viable alternative to this if you can get accurate repeatable open height.
