OK I've been sifting through hundreds of posts and I cant find the info i need so here goes. I'm trying to tally up the causes and makes/models that are a problem. It seems the puck style XTD/Centreforce cheap clutches are getting a bad rep. Having discussed clutches with a few local clutch specialists all have said:
Your flywheel NEEDS refacing with a new clutch if using a ceramic clutch.
Your flywheel NEEDS refacing with a normal clutch if its had a lot of miles as the step-height will be reduced.
You HAVE to get the friction surface AND bolt/dowel surface resurfaced by the same quantity or you will get slip almost from the get-go.

If youve had a failure please name and shame the clutch and be honest about whether you refaced/replaced the fly at the same time. If you had it resurfaced first are you sure the guy doing it did maintained the stock spec step height?

So the question is posed...

I had a failure of my spec stage III. Clutch springs came out and destroyed my clutch and flywheel (fidanza at the time with new surface).

I installed a new exedy clutch and a pressure plate w/ 30% more holding force. I resurfaced the flywheel before install. So far ive put about 70K on the clutch without a glitch.

For those that want a major deal and a slightly better than stock clutch:

C.A.P. clutch for $170 total. Includes: exedy clutch, pressure plate (30% more holding), throw-out bearing and alignment tool. I've only lauched once but it held really well.

Just been on their site and its got some good prices i have to admit! Wonder if they do aftermarket flywheels :)

I"ll echo the Exeddy.
I wish I had stuck with mine....

I've had trouble with a stage III Clutch Masters going at roughly 25k miles.
Installed it with a brand new CM flywheel, which was checked before it went in.
One of the springs in the disc broke out.

I installed a Exeddy stage zero disc with the CM PP.
Worked GREAT for 6 months and several thousand miles I had it in.
6K rpm launches and all. It hardly showed any wear when I pulled it out.

Unfortunately my dumbass chucked it in favor of a Spec stage II dics/PP.
Brand new TO bearing and resurfaced flywheel.

5 Months and less than 8k miles later [no hard launches, ran it a bit hard once the clutch engaged. Broke in before hand] it broke. I still havn't torn it down to see what happened, but I suspect the disc again....

I'm sticking a remaned OEM disc with the Spec PP [assuming it's good] one of these days.
